About the Role

As a Care Navigator, you'll be the first point of contact for patients, carers, and healthcare professionals-providing compassionate, efficient, and professional support over the phone.

Key Responsibilities:

Handle incoming calls for GP practices, ensuring patients are directed to the most appropriate clinician or service.
Escalate emergency calls promptly to ambulance services.
Support the wider primary care team with follow-ups and coordination.
Accurately record patient information using the EMIS Web system.
Liaise with external services and follow clinical instructions.
Maintain confidentiality and uphold a patient-first approach.
Navigate local referral pathways to ensure holistic care
What We're Looking For

Strong verbal and written communication skills.
Ability to work independently and as part of a team.
Proficiency in Microsoft Office and EMIS Web (essential).
Experience in a primary care setting and call handling.
Empathy, patience, and professionalism in every interaction.
