About the Role

As a Care Navigator, you'll be the first point of contact for patients, carers, and healthcare professionals-providing compassionate, efficient, and professional support over the phone.

Key Responsibilities:

• Handle incoming calls for GP practices, ensuring patients are directed to the most appropriate clinician or service.
• Escalate emergency calls promptly to ambulance services.
• Support the wider primary care team with follow-ups and coordination.
• Accurately record patient information using the EMIS Web system.
• Liaise with external services and follow clinical instructions.
• Maintain confidentiality and uphold a patient-first approach.
• Navigate local referral pathways to ensure holistic care

What We're Looking For

• Strong verbal and written communication skills.
• Ability to work independently and as part of a team.
• Proficiency in Microsoft Office and EMIS Web (essential).
• Experience in a primary care setting and call handling.
• Empathy, patience, and professionalism in every interaction.
