Main duties of the job
The Patient first contact is predominantly via Accurx, our online triage system, ensuring timely, efficient and appropriate allocation of care across the multidisciplinary team.
Clinical CareProvide high-quality, personalised medical services to the practice population, delivering sensitive, responsive and patient-centred care.
Ensure that ethnic, linguistic, cultural and social needs of patients are recognised and appropriately addressed in all aspects of care.
Service DevelopmentContribute to shaping, developing and organising services to reflect the evolving needs of the local community and practice population.
Learning & EducationPromote and contribute to the practice as a learning organisation, maintaining up-to-date knowledge of current policy, clinical guidelines and best evidence, and supporting their integration into everyday practice.
Team Working & LeadershipWork effectively within the primary healthcare team, fostering strong multidisciplinary collaboration and taking on leadership responsibilities where appropriate.
Quality Improvement & GovernanceActively participate in and promote quality improvement initiatives, including clinical governance, audit, and service evaluation.
Practice Values & ObjectivesWork in line with the practices aims, objectives and values, contributing to a positive, supportive and sustainable working culture.

Our Practice family is large, diverse and highly supportive, consisting of7 GP's, GP Registrars, ANP, Nurse Prescribers, Practice Nurses,Pharmacists, Community Paramedic, and HCA's. Our strong administrative and management team play a vital role in reducing GP workload and supporting efficient service delivery, including 17receptionists and a head receptionist at each location, 3 clinicalcoders, one scanner, data quality lead, 2 deputy Practice Managers, Office Manager and Practice Manager.
In addition to our core services, we are a committed and active member of our local Primary Care Network (PCN). We work collaboratively with PCN partners to improve patient access and service delivery across our local area.

Role SummaryThe post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all patients.
Clinical ResponsibilitiesIn accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork (docman) and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Recording clear and contemporaneous consultation notes to agreed standards.
Screening patients for disease risk factors and early signs of illness.
Developing care plans for health in consultation with patients and in line with current practice disease management protocols.
Providing health education.
Compiling and issuing computer-generated acute and repeat prescriptions.
