Are you an experienced practice nurse looking for a new challenge?

Our nursing team play a fundamental role in the practice, providing skills and expertise valued by the team and wider community. We are currently looking for an enthusiastic and forward-thinking nurse to join our team of 6 practice nurses, working 30- 37.5 hours per week.

Ideally, we are looking to appoint a GPN with experience in long term condition management, particularly diabetes, and a non-medical prescriber.

One of the underpinning values of our practice is training and development. The nurses are encouraged to progress with training and skills to benefit their personal development and that of the practice.

About us

Beacon Primary Care is an accredited training practice; we provide education for GP trainees and university students. Our team consist of experienced trainers committed to mentoring the next generation of clinicians.

We are a busy, friendly expanding two-partner practice, currently caring for a population of 17,000 patients across four sites in West Lancashire.

Main duties of the job/key responsibilities:

  • Cervical cytology
  • Chronic Disease Management to include but not limited to:
    Asthma, Diabetes, COPD including spirometry, CHD and LD
  • Routine procedures including wound care and basic dressings
  • Vaccination and Immunisation for adults and children including travel health
  • Prescribe within competency of role & responsibilities

Our multidisciplinary team
consists of :

  • 2 GP partners.
  • 7 GPs, GP Registrars, ANP, Nurse Prescribers, Practice Nurses, Pharmacists, Community Paramedic, HCA's, MHP, FCP and Social Prescriber.
  • Our administrative team includes - Practice Manager, 2 Deputy Managers, Data Quality Manager & Office Manager. The team are supported by 17 Receptionists which include a Head Receptionist at each location, Clinical Coders and Scanners.

We are an innovative practice who continuously strive towards improving the quality of care we deliver. We participate in all national and local enhanced services and offer our staff a wealth of training opportunities.

Our practice study days are hosted on a regular basis, and when possible include the whole team. We have a robust meeting schedule to ensure governance is addressed regularly.

Job responsibilities

Beacon Primary Care

The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population across all Beacon Primary Care sites. The post holder is accountable for contributing to the nursing service delivery. Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients.

As autonomous practitioners the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making. They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership as required.

Key responsibilities

Clinical practice

  • Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being
  • Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
