Licensed Practical Nurse - Full Time - Sign On Bonus

We are immediately hiring for the Licensed Practical Nurse (LPN) role at Stratford Manor Rehabilitation & Care Center
.

Full‑time, part‑time, and per‑diem shifts are available: 7 am–7 pm (FT) and 7 pm–7 am (FT).

New nurse graduates and LPNs with experience are welcome! We provide comprehensive training, orientation, and support in a collaborative environment led by an engaged management team.

• Assist CNAs in performing ADL and routine care.
• Conduct resident/patient treatments.
• Administer medication in accordance with physician orders and the Plan of Care.
• Submit pharmacy orders.
• Accurately document and record all information.
• Give injections of medication as prescribed.
• Observe resident health and communicate current condition to RNs, supervisors, and physicians.
• Take vital signs, including blood pressure, temperature, and weight.
• Provide basic care, including wound cleaning and bandaging.
• Manage IVs, start IV drips.
• Monitor fluid/food intake and output.
• Observe and report resident accidents, incidents, and skin breakdowns.
• Assist as directed in the admissions process.
