Job Summary
Bolton Community Mental Health Services are a committed integrated service that strives to provide quality recovery‑based support to people with severe and enduring mental health‑related difficulties in Bolton 7 days per week, 365 days per year. As a Community Psychiatric Nurse you will act as Care Coordinator in line with CPA process, manage a complex caseload, work proactively and flexibly within a multi‑disciplinary team, and work outside normal office hours.
Mainduties of the job
- Provide skilled mental health nursing care in the community.
- Assume the role of care coordinator for a complex caseload.
- Work as a member of a multi‑disciplinary team.
- Maintain knowledge of the Safeguarding Adults and Children framework and take lead when necessary.
- Conduct initial assessments of people referred into the service, linking with the Team Leader and wider MDT.
- Develop, implement and evaluate holistic programmes of care, including carer assessment.
- Maintain close links with statutory and non‑statutory professionals and agencies, accessing resources from provider services and ensuring good communication channels.
- Act in accordance with the philosophy of the Care Programme Approach and related legislation.

- Registered Mental Health Nurse – Diploma in Mental Health Nursing or Degree/BSc in Mental Health Nursing.
- Demonstrable post‑qualifying experience.
- Continued professional development in accordance with the Nursing and Midwifery Council requirements.
- Ability to use initiative to deal with situations which may occur.
- Collaboration skills with colleagues from other disciplines.
- Good verbal and written communication skills.
- Up‑to‑date clinical & professional practice, delivering appropriate interventions and assessing mental health needs, risk profiles and planning, delivering and evaluating care based on outcomes in line with Trust and local policies.
- Experience of working within a community setting.
- Experience with in a community‑based mental health team (CMHT).
