Overview
Bolton Living Well currently have a full-time post available on a fixed-term or secondment basis for a maximum period of 12 months. We are looking to recruit a Registered Mental Health Nurse, Registered Social Worker or Occupational Therapist to fill this role.
Responsibilities- Undertake a key worker role, alongside an assessment and formulation role within the team.
- Triaging and signposting referrals; liaising with a number of agencies and professionals.
- Based in Farnworth or Horwich (subject to clinical need) and participate in a Duty capacity on a rota basis, supporting the new Referral and Assessment Hub at Royal Bolton Hospital; may be rostered to work until 7pm and occasional weekend working as the service develops.
The Living Well Service enables timely access to mental health services when people need it most and provides support for people who might not traditionally meet the threshold or criteria of existing mental health services. The team is multidisciplinary and is operated in partnership with Family Action (BAND), our third sector partner. Bolton Living Well Service is compassionately-led, trauma-informed, and recovery-focused.
CPD opportunities are available and personal interests/skills are supported to develop and grow.
The postholder will be based in either of our Hubs, in either Farnworth or Horwich and subject to clinical need. The service operates from 9am to 5pm Monday to Friday; staff may be required to participate in a Duty capacity on a rota and may include evening, weekend work as the service develops.
Experience and qualifications- Experience of triaging and signposting referrals; ability to liaise with a number of agencies and professionals.
- Experience and knowledge of community services and third sector organisations in Bolton is beneficial.
