This is an exciting opportunity for a Registered Mental Health Nurse to join our specialist service for people experiencing a first episode of psychosis as a Community Psychiatric Nurse/ Care Coordinator. You will be joining a client centred, dynamic and friendly multi‑disciplinary team, who prides themselves on delivering high quality, evidence‑based interventions for those experiencing first episode psychosis. Based on the Department of Health implementation of NICE guidelines, Early Intervention Services aim to reduce the stigma associated with psychosis, improve awareness of the symptoms of psychosis and the need for early assessment, reduce the length of time young people remain undiagnosed and untreated, develop meaningful engagement, provide evidence‑based interventions and promote recovery during the early stage of illness, increase stability in the lives of service users, facilitate development and personal fulfilment, provide a user‑led service, and ensure that care is transferred thoughtfully and effectively at the end of involvement.

Responsibilities
  • Utilise highly developed interpersonal skills including specific skills to facilitate effective communication when there are barriers to understanding e.g. sensory or cognitive impairment or in a hostile, antagonistic or highly emotive situation.
  • Establish and maintain robust communication networks with clients, carers, and other health care workers and numerous external agencies.
  • Maintain good working relationships with GPs, GP commissioners and relevant consortia.
  • Promote awareness of the role of Early Intervention within local service area/team, GP practices and the wider community.
  • Complete specialist assessments for referred service users accepted into the service when required.
  • Be a full member of the multi‑disciplinary team (MDT).
  • Maintain a high standard of clinical record keeping, including electronic data entry, report writing and exercise of professional self‑governance in accordance with relevant professional code of practice and Trust policies and procedures.
  • Contribute to the team adherence to the NHS England Access and Waiting Time Standards for Early Intervention Psychosis.
  • Undertake risk assessment and risk management for individual service users and provide advice to other professionals on acute and chronic aspects of risk assessment and risk management.
  • Liaise professionally and sensitively with other involved parties to provide seamless and consistent high‑quality care for service users under the team’s care.
  • Hold and manage a caseload of service users experiencing their first episode of psychosis, including care planning and delivering psychosocial interventions.
  • Have a good knowledge of required documentation for case management and ensure compliance with any associated performance standards.
  • Undertake and record comprehensive risk assessment, devise and implement risk assessment plans for service users at risk of vulnerability, self‑harm, self‑neglect or risk of physical, sexual, emotional harm to others, using the CRASP risk assessment tool.
  • Be aware of and adhere to responsibilities for people under the Children’s Protection Act, including offering/arranging carer support.
  • Be aware of and adhere to the NICE guidelines for first episode psychosis.
  • Manage discharges from the service and transfers to other services as appropriate for service users who have reached the end of their time with the service.
  • Work as part of the integrated MDT and participate in clinical decision‑making to determine the most appropriate management plans for all service users under the team’s care.
  • Provide advice, guidance and consultation to peer and junior professionals within the service, individually and by attendance at team referral and review meetings.
  • Participate in the duty/assessment rota operated by the Early Intervention Team.
  • Receive regular clinical and managerial supervision.
  • Remain informed of current research and guidance in the field, support own professional work and contribute related knowledge to the MDT.
