A vacancy at Greater Manchester Mental Health NHS Foundation Trust. We are looking to recruit three Mental Health Practitioners to support the screening of referrals and subsequent assessments within the Referral and Assessment Hub in Bolton.

Role Overview

As part of Community Transformation, you will screen referrals to the Hub for people with mental health difficulties. Following your screening/assessment as a Trusted Assessor, you and the multidisciplinary team (MDT) will determine the care pathway for the service user, ensuring allocation to the most appropriate service, including the VCFSE sector and Social Care.

You will provide duty assessments at the designated base or at a venue agreed with the service user and carer, and you must be able to travel across Bolton.

These posts are full‑time positions with working hours 9 am – 7 pm, Monday to Friday initially, over a flexible rota. Depending on service need, the service may extend to cover seven days per week, including Bank Holidays.

Key Duties
  • Triage referrals entering the Hub with the MDT, providing onward journeys for those not suitable for community services.
  • Maintain competent knowledge of community resources available outside statutory services to support people in mental health recovery.
  • Link in with the MDT and attend the daily meeting to discuss assessments.
  • Promote awareness of the Referral and Assessment Hub within the local service area, team, GP practices, and the wider community.
  • Support service user, carer, and stakeholder involvement.
  • Promote a culture of learning, education and professional development.
  • Adhere to safeguarding principles for children and vulnerable adults in line with Trust policy and local reporting procedures, ensuring junior staff compliance.
