CCS is home to Caledon’s only accessible transportation program, the Exchange Community Hub that includes food support and a range of activities, the Caledon Specialist Clinic, and personal support services for seniors. We offer settlement services and language classes for newcomers and comprehensive employment services for job seekers and local businesses. We provide paid training programs for Caledon youth as well as exciting internships and volunteer opportunities.

Position Overview:

The Contract Community Supports Case Worker provides timely and effective social services to clients through a comprehensive needs assessment process that identifies client need and responds with the provision of practical supports, service systems navigation and referrals, and working in collaboration with other service providers and community partners.

Position Deliverables/Accountabilities:

Under the general direction of the Manager, Food Bank, the incumbent:

  • Engages clients in the completion of a program needs intake and assessment;
  • Develops relationships with clients to facilitate effective service delivery;
  • Oversees the distribution and administration of subsidy and basic support services such as Help with Heat, Season of Hope, Food Support, CARE, etc., referrals and other available subsidies;
  • Supports clients to navigate social service systems and complete required documentation as needed;
  • Reception coverage and client/guest receiving as required;
  • Collaborates with other service providers to promote timely referral and coordination of client needs;
  • Monitors and reviews client files regularly, adjusting when required to ensure client needs are being met;
  • Keeps accurate and confidential records to track referrals, service provider consults and feedback on progress;
  • Maintains comprehensive statistical data and other information or reports as required, to meet funder criteria;
  • Supervises volunteers and student placements assisting with the provision of social services;
  • Builds working relationships with service providers both within CCS and in the broader community through ongoing communication, collaboration and advocacy regarding client needs;
  • Provides support and assistance with Exchange programming as required;
  • Engages in outreach strategies to broaden client base;
  • Participates in team meetings;
  • Other duties as required
