Citizens Memorial Healthcare Facility CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. About the Facility:This skilled nursing facility is a111-bed facility located in Bolivar, MO.
A typical day includes: assigning care duties to nursing personnel consistent with their training and experience. The LPN is responsible for safe medication administration, conducting resident assessments, monitoring changes in Activities of Daily Living (ADLs) and administering treatments and procedures per physician order.
- Developing relationships with our residents
- Facilitating resident admissions and discharges
- Assisting in the development of the resident's Plan of Care
