Citizens Memorial Hospital (CMH) is recruiting an OB RN Acute Care Manager to help care of our postpartum and post‑surgical patients. CMH is made up of 34 primary and specialty clinics, 6 Long‑Term Care facilities, and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk‑in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!

Acute Care Management facilitates the care of our inpatients from admission through discharge in collaboration with the associated care team, ensuring progression of stay and efficient utilization of needed resources, individualized to patient needs.
A typical day includes:This OB ACM RN will support patients both inside and outside the four walls of the hospital through safe discharge planning and post‑discharge follow‑up phone calls, and support departmental quality measure data collection for this OB patient population.
- Focusing on patient education and validation of patient understanding
- Ensuring patient understanding of their responsibilities in managing their health after discharge
- Promoting quality patient care outcomes
- Education:
Associates or Bachelors - Experience:
Requires OB experience - Skills:
Strong communication and interpersonal relationship development skills are critical - Licensure/Certification: RN
