The Nurse Navigator coordinates the delivery of patient care by assessing care requirements of the individual patient and making recommendations based on severity of illness, intensity of service, and skill level. They ensure patients receive timely, safe, effective, and high‑quality care and coordinate the patient’s plan of care with bedside nurses. The Nurse Navigator sets expectations regarding professional conduct and serves as a resource for the patient care unit, holding direct and indirect care providers accountable for their responsibilities and adherence to regulatory guidelines.
This position is responsible for managing the assigned inpatient and outpatient populations and facilitates a new care model—supporting quality patient care, safety, and financial components; promoting seamless integration; coordinating patient discharge and utilization management; and collaborating to optimize performance and best practices. Responsibilities include pre‑op education, daily hospital rounding, patient discharge disposition, and follow‑up care.
- Location: Mercy Clinic Springfield – Gastroenterology Whiteside
- Schedule: Full‑Time, Monday – Friday
The Nurse Navigator is a nursing specialist who follows patients across the continuum of care, coordinating with an advanced level of clinical expertise and collaborating with Mercy’s patients, families, physicians, interdisciplinary team, and other support staff.
Qualifications- Experience: 2 years related experience
- Required
Education:
Graduate from an approved school of nursing - Preferred Education: Bachelor of Science in Nursing
- Certifications: RN (Registered Nurse) licensed in the state of practice; BLS (Basic Life Support) required within 30 days of hire
