Citizens Memorial Hospital (CMH) in Bolivar, MOis hiring a Assistant Diabetes Educatoror Registered Nurse to join our staffto care for patients in the Diabetes Education center ! CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!

About the department: CMHdietitians provide various services to help patients improve their nutritional intake. They tailor services to each person's needs.

A typical day includes: Assisting in the education of diabetic patients, performance improvement and quality assurance initiatives, data input and tracking for the diabetes education program.

• Providing education to appropriately referred outpatients, in-patients, home care patients; assisting in continuous quality improvement activities, outcomes tracking, in-service for department and the CMH system; provides community education as indicated.
• Demonstratinghow to interpret and incorporate lab values, anthropometrics, medications, age and medical condition when assessing and planning for diabetes self-management education.
• Maintainingall required documentation; assisting with the development and implementation of program operations and materials; and obtainingfollow-up information for patients participating in the diabetes education program and providingto the Diabetes Educator.
• Some night and weekend times may be required;

Job Requirements:

• Education: Bachelors Degree in Dietetics and Completion of a dietetic internship; or coordinated undergraduate program;or Registered Nurse
• Licensure & Certifications : Registration from Commission of Dietetic Registration;
  
  Licensed Dietitian in the state of Missouri; or Registered Nurse licensed in the state of Missouri;
• Experience:Minimum of one (1) year experience in diabetes self-management education OR Endocrinology clinic

The position is: PRN, rotating; hours may vary.
