Position: Community Health Worker- Full Time

Job Summary

The Community Health Worker works within the Care Team to facilitate patient goals, address Social Determinants of Health, and connect patients with needed resources in the surrounding community. Behaves in a manner consistent with the mission of OCHC and performs other duties as requested.

Education & Training
  • High School diploma or equivalent
  • Must maintain required CEUs
Licensure & Certifications
  • BLS within 6 months of employment
  • CHW Certification within 1 year
Experience
  • One year clinic office experience preferred
Job Responsibilities
  • Provides community-based support services for patients and their families in accordance with OCHC policies and protocols
  • Assists patients and families in navigating, and acts as liaison to, the foster care system, health care systems, Medicaid, DFS/DSS, insurance companies, and others
  • Assists patients with applications, to include MCD, SNAP, LIHEAP, and others
  • Facilitates scheduling appointments for patients, including coordination of transportation
  • Works with patients and families on setting goals related to their health and social needs, and works toward improving self-management competency
  • Advances patients  health literacy related to their conditions. Provides education pertinent to patients  chronic conditions and lifestyle choices
  • Maintains a good working relationship with community agency contacts and school personnel
  • Makes home visits based on referrals from providers, nurses, care team members, school officials, and others
  • Consults with providers and other team members regarding the comprehensive health care of patients and families to coordinate care
  • Makes referrals and connections to community and clinical resources to help patients and families meet basic social and medical needs
  • Follows up with PRAPARE surveys and addresses other barriers to care
  • Keeps detailed, accurate, and timely documentation of all patient interactions and on-behalf activities
  • Monthly reports are accurate and timely
  • Participates in department meetings, trainings, and activities
  • Provides crisis intervention as needed, and appropriately documents hotline calls
  • Participates in ongoing training to develop professionally and meet CEU requirements
  • Other duties as needed
