Home Health Registered Nurse; RN

We are seeking a Part-Time/PRN Registered Nurse (RN) to provide high-quality, in-home care to patients across far South and Western suburbs of Chicago, specifically Aurora, Naperville, Plainfield, Bolingbrook, Minooka, Channahon, Joliet, and Lockport. • Conduct in-home nursing assessments and ongoing patient evaluations
• Perform initial start-of-care and re-certification visits as needed
• Accurately complete OASIS documentation
• Provide Skilled nursing care including wound care, IV line care, and maintenance
• Educate patients and caregivers on disease processes, medications, and plans of care
• Monitor patient progress and promptly report changes to physicians
• Coordinate care with therapists, HHAs, and other disciplines
• Supervise HHA field staff as appropriate
• Ensure compliance with Medicare, agency policies, and clinical standards
• Complete documentation and paperwork in a timely, efficient manner

• Active Registered Nurse (RN) License
• Home Health experience preferred, but not required
• OASIS Documentation experience required
• Strong assessment, organization, and documentation skills
• Strong communication skills
• Ability to work independently in a home-based setting
• Professional, dependable, and patient-focused approach
• Physical ability to perform in-home visits
