- Other duties as assigned.
- Develop professional and clinical skills through attendance at staff development meetings, pertinent educational activities, and professional affiliations.
- Participate in the review process and the agency performance improvement program.
- Provide patient education, medication administration, wound dressing changes, and ostomy care as directed by the RN and ordered by the physician.
- Display flexibility in accepting assignment changes and participate in weekend/after-hours coverage and call rotation.
- Report pertinent observations to the attending physician and RN, and implement changes in the patient care plan as directed.
- Communicate changes to the appropriate members of the home health care team.
- Demonstrate effective communication skills with all team members, patients, and families.
- Maintain clinical record information in accordance with established standards and within established time frames.
- Provide direct patient care responsibilities for assigned patients in an efficient, organized manner under the direction of the RN.
- Perform treatments within scope of training and administer medications to assigned patients.
- Assess patients during clinical check-in, document pertinent medical information and vital signs, and assess for signs of abuse or neglect using age-specific guidelines.
- Administer treatments, medications, and follow-up assessments ordered by the provider, verifying as needed, and respond appropriately in clinical emergencies and triage situations.
- Perform diagnostic and occupational medicine testing, including EKG, spirometry, audiometry, titmus, and pulse oximetry, and complete forensic testing collections in accordance with established guidelines.
- Collect data specific to the age of the patient and contribute to their plan of care.
- Receive, verify, and implement physician orders, ensuring orders taken by the LPN are co-signed by an RN.
- High School Grad or Equiv (Required)
- Technical/Vocational School (Required)
- Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement
- Driver's License (DL) - EV Accredited Issuing Body
- IV Certified (IV) - EV Accredited Issuing Body
- Licensed Practical Nurse (LPN) - EV Accredited Issuing Body
