The Nurse Navigator plays a critical role in ensuring timely, comprehensive, and patient centered coordination of care for individuals seeking evaluation and treatment within the Colon and Rectal Surgery Clinic in Boise. The Nurse Navigator serves as a patient advocate and liaison, guiding individuals and families through the complexities of cancer care. This role is essential in ensuring timely access to care, coordinating multidisciplinary services, and providing education and emotional support throughout the cancer journey.

• Full-time (1.0 FTE) | 80 hours per pay period
• Schedule:
  
  Monday-Friday
• 5 days x 8-hour shifts a week
• No nights, weekends, or St. Luke’s observed holidays
• Sign-on bonus eligible for new hires!
• Certification pay available (ex. OCN, CNOR, CGRN)
• Assess barriers to oncology care and offers support, care coordination, guidance and education for patients and families with a new diagnosis of cancer
• Responsible for the clinical review of incoming referrals, triage of patient needs based on urgency and complexity, patient outreach and education, and facilitation of appropriate scheduling and care transitions
• Serve as key liaison between patients, referring providers, surgeons, advanced practice providers, and multidisciplinary care teams

  Nursing Degree
• Experience:
  
  Two (2) + years’ RN experience
• Licenses/Certifications:
  
  Current RN License in the State of Practice AND Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross
• Preferred: colorectal, GI, oncology, or surgical specialty settings
