Position: Nurse (Domiciliary Admission Coordinator)
The Domiciliary Admission Coordinator Registered Nurse (RN) executes position responsibilities that demonstrate leadership through the delivery of improving holistic care, experiences, and participating in creative approaches to managing complex patient care through collaborative strategies with others. The nurse is responsible for providing competent, evidence-based care to assigned patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting.

General Description of Assigned Duties:

* Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.

* Assesses, plans, implements, and evaluates care based on age-specific components.

* Coordinates care with a focus on patient education, self-management, and customer satisfaction throughout the continuum of care.

* Administers medications and procedures in accordance with established policies and guidelines.

* Influences care outcomes through collaboration with members of the interdisciplinary team.

* Executes responsibilities that demonstrate leadership, experience, and creative approaches to managing complex client care.

Functions or Scope of Assigned Duties:

* Serves as an active member of an interdisciplinary team assisting with patient planning, assessments, identification of barriers, and activities required for successful transition into the residential treatment setting.

* Applies the nursing process, including systematic data gathering, assessment, and appropriate nursing judgment to ensure positive patient outcomes.

* Coordinates and collaborates with interdisciplinary teams, VA and non-VA departments, community agencies, and healthcare providers to address patient needs.

* Participates in the development, implementation, evaluation, and revision of policies, procedures, and guidelines.

* Assists in directing nursing education, orientation, competency development, and quality improvement initiatives.

* Provides patient-centered and recovery-oriented care for individuals with substance use disorders and/or mental health conditions.

* Facilitates shift reports, warm hand-offs, and nurse-led psycho educational groups.

* Utilizes therapeutic communication techniques, de-escalation strategies, and recovery-oriented language.

* Oversees safe medication management programs and promotes critical thinking in clinical practice.

* Participates in patient safety and environment of care rounds; directs team response to psychiatric and medical emergencies; reports and resolves safety concerns.

* Documents nursing assessments, risk screens, measurement-based care tools, nursing care plans, and safety/environment rounds per facility policy.

* Maintains professional boundaries to protect patient vulnerabilities and act in the best interest of the patient.

* Provides peers with constructive feedback and supports colleagues through knowledge sharing to ensure safe, quality nursing care.

* Fosters a safe and supportive environment conducive to professional development.

* Demonstrates leadership performance broad enough to improve care for a group of patients.

* Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.

* Responsible for documented outcomes at the program or service level, which must be broad, complex, and demonstrable at any organizational level within the facility.

Pay:
Competitive salary, regular salary increases, potential for performance awards

Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Retirement:

Licensure: 1 full and unrestricted license from any US State or territory

Work Schedule:

Monday - Friday; 6:30am - 3:00pm or 7:00am - 3:30pm
