Position Summary

The Licensed Practical Nurse (LPN) provides skilled nursing care to hospice patients in their homes under the supervision of a Registered Nurse and according to physician orders. You will deliver care in alignment with hospice standards, your scope of licensure, and established clinical practices. This position reports to the Director of Nursing (or designee).

What You'll Do
  • Observe, assess, document, and report patients' physical and mental conditions
  • Communicate changes in patient condition to the RN, Case Manager, Team Leader, or physician
  • Assist in developing and carrying out individualized plans of care
  • Administer medications and treatments in compliance with state law and nursing standards
  • Provide patient and family education, emotional support, and compassionate care
  • Complete accurate and timely clinical documentation for each visit
  • Assist RNs with specialized procedures as needed
  • Participate in on-call rotation, including evenings and weekends
  • Collaborate closely with the interdisciplinary hospice team
  • Support hospice aides during visits when appropriate
  • Respond to patient, family, and staff needs, including after-hours support
What We're Looking For
  • Active LPN license in the state of practice
  • At least 1 year of nursing experience (home health or hospice experience preferred)
  • Current CPR certification
  • Strong verbal and written communication skills
  • Ability to work independently while collaborating with a care team
  • Reliable transportation, valid driver's license, and current auto insurance
  • Flexibility to travel and adjust schedule as patient needs arise
