Purpose
We are looking for a Community Health Worker to support our clinic.
OverviewThe Community Health Worker/Certified Enrollment Counselor (CHW/CEC) reports to the Community Outreach Program Manager (COPM). The CHW/CEC is an important team member supporting the Outreach & Enrollment Department, which is essential for achieving the dual mission of Full Circle Health. The CHW/CEC shall be accountable for:
- Professionally and compassionately serve to connect the community in the Family Medicine Health Center (FMHC) services area to local healthcare and community services and resources.
- Responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors.
- Provide social support and informal counseling, advocate for individuals and community health needs. Community outreach, such as home visits and helping with health screenings at community fairs will be required.
The CHW/CEC works closely with receptionists, providers, nurses, medical assistants, counselors, dietician, and referral coordinators as well as team members from the pharmacy and business office departments.

- Medical and/or social service experience preferred but not required.
- Spanish language is preferred but not required.
