- Trinity 10 Panel Drug Screen completed within 30 days of start date, preferred within 14 days of start
- Trinity Code of Conduct Form signed
- Trinity Confidentiality of Patient and Proprietary Information and Information System Security Agreement signed
- Trinity Integrity and Audit Services Form signed within days prior to start
- Trinity Motor Vehicle Report (MVR) must be dated within 30 days of start
- Trinity Color Vision test required
- COVID-19 Vaccine proof of vaccine or declination/exemption required
- MMR proof of 2 vaccinations or IgG titer required
- Hepatitis B vaccination series required prior to start
- Influenza Vaccine proof or declination/exemption required

- Trinity NSO report dated within 30 days of start required
- Trinity System Access Form must be completed and uploaded one week before start
- Trinity Skills Checklist for clinical contractors
