Overview
LPN Clinical Care Team Coordinator role at New York Cancer & Blood Specialists (NYCBS) – Bohemia, NY.
Hours:
Monday–Friday (8.5hr shifts).
- Monitor and manage patient communications, ensuring timely responses and appropriate action on messages related to patient care coordination.
- Patient outreach and education and clinical questions within the LPN scope of practice.
- Guide patients through their care journey by facilitating appointments, follow-ups, and ensuring access to necessary services.
- Assist centralized FMLA team with completion of forms requiring provider signature.
- Process and coordinate prescription refill requests in collaboration with healthcare providers, ensuring accuracy and timely delivery.
- Utilize Prescription Monitoring System (PMP) for refill requests of controlled substances, ensuring proper follow-up is scheduled.
- Liaise with the revenue cycle team to provide patients with updates on authorizations and referrals.
- Arrange Peer to Peer conversations with providers to ensure authorization of testing and medications is completed in a timely manner.
- Work closely with physicians, nurses, and other healthcare professionals to develop and implement individualized plans of care for the patient.
- Identify qualifying patients and enroll in Chronic Care Management to support ER avoidance.
- Coordinate care for patients experiencing acute symptoms with the triage team and APP.
- Serve as a liaison between patients and the healthcare team, addressing concerns and ensuring patient needs are met promptly.
- Educate patients on their healthcare needs to involve them in their overall treatment plan.
- Maintain accurate and up-to-date patient records, documenting all interactions, care time, and outcomes in compliance with health regulations.
- Graduate from an accredited School of Nursing and hold an unencumbered New York license.
- Nursing experience with clinical competence preferred.
- Excellent assessment, communication and organizational skills.
- Good computer knowledge.
- Ability to plan, organize, develop, and interpret health data, goals, and objectives necessary for providing quality coordination of care.
- Bi-lingual English/Spanish a plus.
- Hem Onc experience preferred.
- Salary:
Starting at $32/hour based on experience. - Benefits:
