We are looking for a highly motivated, patient centred Social Prescriber, working within the 3 Harbours and Bosvena neighbourhood.

Working within our multi-disciplinary healthcare team, your role will be providing 1:1 personalised support to people, with an emphasis on children, young people and families, who are referred to you by team members and local agencies.

This post will be practice based working across our Primary Care Network geography including Bosvena (Bodmin), Lostwithiel, Middleway and Fowey River Practice.

Main duties of the job

This post empowers people to take control of their healthand wellbeing by giving time to focus on what matters to them. You will buildtrusting relationships with people, create personalised plans and connect people to community groups and other voluntary and social enterpriseorganisations and services. You will also work with partners to provide supportto community groups and Voluntary, Community and Social Enterpriseorganisations (VCSE) involved in social prescribing.

This role helps people to work on their healthand wellbeing, specifically wider determinants of their health, such as debt,poor housing and physical inactivity, as well as other lifestyle issues andlow-level mental health concerns, by increasing peoples active involvement withtheir local communities. This approach particularly helps with long term conditions (including support for mental health), people who are lonely orisolated, or who have complex social needs which affect their wellbeing.

You will be passionate about reducing health inequalities, especially in support of those people, and families in hard to reach places who struggle to engage with health and care services. Ideally you have a background working with children, young people and families and relish the prospect of building relationships with patients and system partners to support improving outcomes.

About us

3 Harbours & Bosvena Primary Care Network (PCN) is based in mid-Cornwall.

We have a population of 43,500 patients and a large collaborative multi-disciplinary team working across the PCN. Our social prescriber team covers neighbourhood and includes a Lead, and GP Clinician for supervision and mentoring.

Job responsibilities

Provide 1:1 health support to children, young people, adults and families.

Promote social prescribing, its role in self-management, andthe wider determinants of health.

As part of the PCN multi-disciplinary team, build relationships with staff in GP practices within the local PCN, attendingrelevant MDT meetings, giving information and feedback on social prescribing.

Be proactive in developing strong links with all localagencies to encourage referrals, recognising what they need to be confident inthe service to make appropriate referrals.

Work in partnership with local agencies to raise awareness of social prescribing and how partnership working can reduce pressureon statutory services, improve health outcomes and enable a holistic approach to wellbeing.

Provide referral agencies with regular updates about social prescribing, including training for their staff and how to access information to encourage appropriate referrals.

Seek regular feedback about the quality of service andimpact of social prescribing on referral agencies.

Be proactive in encouraging self-referrals and connectingwith all local communities, particularly those communities that statutoryagencies may find hard to reach.

Meet people on a one-to-one basis, making home visits where appropriate within organisations policies and procedures. Give people time totell their stories and focus on what matters to me. Build trust with theperson, providing non-judgemental support, respecting diversity and lifestylechoices. Work from a strength-based approach focusing on a persons assets.

Be a friendly source of information about health, wellbeingand prevention approaches.

Help people identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, being unemployed, lonelinessand caring responsibilities, connecting them to appropriate solutions.

Work with the person, their families and carers and considerhow they can all be supported through social prescribing.

Help people maintain or regain independence through life skills, adaptations, enablement approaches and simple safeguards.

Work with individuals to co-produce a simple personalised support plan to address the persons health and wellbeing needs based on thepersons priorities, interests, values and motivations including what they canexpect from the groups, activities and services they are being connected to andwhat the person can do for themselves to improve their health and wellbeing.

Where appropriate, physically introduce people to community groups, activities and statutory services, ensuring they are comfortable.

Follow up to ensure they are happy, able to engage, included and receiving good support.
