Roles and Responsibilities:Caregivers
- Schedule and manage staffing to ensure continuity of care and consistently high-quality services for clients.
- Read the client’s Axis Care file thoroughly, including their priority notes and other client notes (Ex: weight and height, pet info, diagnoses, ADL list and attributes etc.)
- Ensure the pay rates are correct and consistent.
- Monitor caregivers clock ins and outs
- Monitor alerts for caregivers who have not clocked in or out and contact them within 12 minutes of receiving an alert.
- Verify all shifts the following morning, including weekends, by confirming accurate clock-in and clock-out times.
- Handle clients’ issues, change and communication in a timely manner.
- All calls should be returned to client immediately
- Prioritize cases daily based on urgency
- Actively participate in scheduled Monday and Friday update meetings to provide feedback on calls, respond to urgent inquiries, and coordinate required assistance.
- Collaborate with and support team members to promote efficiency, accountability, and team success.
- Once core client schedules are completed, assist other coordinators in fulfilling remaining shifts.
- Engage with the client liaison to understand client requirements and care needs.
- Handle on-call duties one to two times per month, ensuring all non-business hours are covered (ad hoc requirements: ER visits, death cases, cancellations, caregiver or scheduling issues).
- Notify the internal team who is on call for the week.
- Monitor and respond to messages received via email, business cell, answering service.
- Enter detailed notes immediately after each communication to ensure all parties are fully informed.
- Verify all pay rates, billing rates, and shift times in real time.
- Notify management promptly of any changes to the designated on-call person during the week or weekend.
- Experience in home care, healthcare coordination, or scheduling preferred.
- Exceptionally organized, with a proven ability to think quickly and adapt under pressure.
- Ability to communicate with clients calmly, clearly, and professionally.
- Proficiency with scheduling software (e.g., Axis Care) and basic computer applications.
- Demonstrates empathy and understanding when engaging with caregivers, clients, and clients’ families.
- Effectively manages time-sensitive tasks and priorities.
- Always maintains a high level of professionalism.
- Demonstrates reliability and accountability through timely communication and prompt return of calls, texts, and emails.
- Clients feel informed, supported, and confident in their care.
- Caregivers understand their responsibilities and receive timely support.
- Schedules and documentation are accurate, complete, and updated consistently.
- Communication with internal teams is prompt and professional.
