• Facility: Boca Circle Rehabilitation Center
• Location: Boca Raton, FL
• Shift: Part-Time | 7:00 AM - 3:00 PM
• Pay Rate: $32-$40 per hour (based on experience)

Boca Circle Rehabilitation Center is seeking a dedicated and compassionate Wound Care Nurse (RN/LPN) to join our clinical team. In this vital role, you will collaborate with an interdisciplinary team (IDT) to deliver high‑quality wound and skin care services, ensuring optimal comfort, healing, and quality of life for our residents.

• Collaborate with the interdisciplinary team (IDT) to plan, implement, and evaluate individualized resident care plans
• Assist with completion of applicable sections of the Minimum Data Set (MDS)
• Review, update, and follow skin and wound care policies and procedures
• Assess, treat, and manage primary and secondary wounds
• Identify and manage risk factors for pressure ulcers and secondary wound infections
• Perform routine skin assessments and assist with pain management related to wound care
• Educate residents and families on wound treatments and care plans in a clear, compassionate manner
• Evaluate nutritional needs of residents with wounds and healing concerns
• Recommend appropriate pressure‑relieving devices, wound dressings, therapies, and products to the IDT
• Promptly report any suspicion of abuse, neglect, fraud, waste, or crime to the Administrator or designee

• Current, active, and unencumbered RN or LPN license in the state of Florida
• Minimum of 2 years of nursing experience; wound care experience preferred
• Strong assessment, documentation, and communication skills
• Experience in skilled nursing or long‑term care is a plus
