Responsibilities
  • The purpose of the Pool RN Case Manager is to support the physician and interdisciplinary team in facilitating patient care, with the objective of enhancing clinical outcomes and patient satisfaction while managing the cost of care.
  • Integrates and coordinates care facilitation and discharge planning functions while partnering with the UR RN and Social Work Case Manager.
  • As the RN Case Manager I, accountable for a designated patient caseload and planning to meet patient needs, manage length of stay (LOS), and promote efficient utilization of resources.
  • Facilitates collaborative management of patient care across the continuum and intervenes to remove barriers to timely and efficient care delivery and reimbursement.
  • Applies process improvement methodologies in evaluating outcomes of care.
  • Coordinates communication with the healthcare team, patients, families and post-acute care services.
  • Performs readmission screening and prevention activities.

Estimated pay range for this position is $45.00 / hour depending on shift as applicable.

Qualifications

Degrees: Associates.

Licenses &

Certifications:

  • Registered Nurse.

Additional Qualifications:

  • RNs hired prior to 2/2012 (10/1/2017 at Bethesda or 7/1/2019 at BRRH) with an Associate Degree are not required to have a BSN to continue their non-leadership role as an RN; however, required to complete the BSN within 5 years of hire.
  • 3 years of hospital clinical experience with 1-3 years of hospital or payor Case management experience preferred.
  • Excellent interpersonal communication and negotiation skills; strong analytical, data management and computer skills.
  • Current working knowledge of discharge planning, utilization management, case management and performance improvement preferred.
  • Understanding of pre-acute and post-acute venues of care and post-community resources preferred.
  • Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components.
  • Ability to work independently and exercise sound judgment in interactions with the health care team and patients/families.

Minimum

Required Experience:

  • 3 years of hospital clinical experience required
  • 1 year of Case Manager experience required
