Salary: Up to $97,760 per year

• Quality Care Oversight:
  Ensure the delivery of exceptional care to our long‑term care residents.
• Comprehensive Assessment:
  Conduct thorough initial interviews and assessments for new residents.
• Care Planning:
  Develop individualized care plans to address residents’ unique needs and goals.
• Interdisciplinary
  
  Collaboration:
  
  Lead care plan meetings with treatment teams, caregivers, and families for coordinated resident care.
• Community Engagement:
  Partner with community specialists to support resident well‑being and recovery.
• Leadership and
  
  Education:
  
  Guide the nursing management team, foster communication across departments, and provide in‑service education on MDS, RAPS, and Triggers.
• Adherence to
  
  Schedule:
  
  Maintain timely completion of the MDS/Care Planning schedule for non‑Medicare residents.

• Active Florida RN or LPN License (in good standing).
• Minimum of 2 years in a Skilled Nursing Facility.
• Strong understanding of MDS, Medicare regulations, and the care‑planning process.
• Excellent organizational, leadership, and communication skills.
