• Develop and implement physical therapy treatment plans for clients
• Provide physical therapy assessment and evaluations
• Work with patients to improve their range of motion and strength
• Educate and counsel patients and their families on physical therapy treatments
• Document all patient care services and progress
• Maintain patient records
• Conduct thorough assessments of patients' physical conditions and functional abilities.
• Develop individualized treatment plans based on clinical assessments and physician recommendations.
• Implement evidence-based physical therapy interventions to improve mobility, strength, balance, and overall functional independence.
• Monitor and document patient progress, adjusting treatment plans as necessary.
• Collaborate with physicians, occupational therapists, and other healthcare professionals to ensure comprehensive and coordinated patient care.
• Provide education to patients and their families on home exercise programs and self-management techniques.

• Licensed Physical Therapist in the state of Florida
• At least 1 year of experience in physical therapy
• At least 1 year of experience working in Home Health environment
• Excellent communication and interpersonal skills
• Strong problem-solving and critical thinking skills
• Ability to lift heavy objects
• Excellent interpersonal and communication skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
