Job Summary:

The NuView Tele Nephrologist is responsible for providing comprehensive nephrology care through telemedicine services. This role involves coordinating care with on-site physicians, accepting patient referrals, and managing all aspects of patient care. The Tele Nephrologist will ensure patients, and their families are well-informed and supported, and will collaborate with various healthcare professionals to deliver high-quality, evidence-based care.

Key Responsibilities
  • Coordinate care with on-site physicians.
  • Accept patient referrals and manage all aspects of care for referred patients.
  • Update patients and their family members, providing supportive and compassionate care.
  • Discuss end-of-life preferences with patients and/or their legal representatives.
  • Consult with sub-specialists and tele-specialists as needed, maintaining a positive co-management model.
  • Practice within evidence-based medicine guidelines, considering healthcare costs and risk-benefit of procedures or therapies.
  • Comply with hospital-based clinical guidelines and initiatives.
  • Facilitate and guide patient recovery and rehabilitation plans.
  • Coordinate care plans with primary care physicians through direct contact and video conferencing.
  • Respond to emergencies during scheduled on-call duties for inpatient and observation patients as requested by medical staff or nursing management.
  • Collaborate with the Hospital's Case Management department to ensure effective utilization and resource management, appropriate medical records, medical necessity, and proactive discharge planning.
  • Maintain legible, accurate, complete, and timely medical records for all patients examined or treated, in accordance with legal, accreditation, regulatory, and third-party payor requirements.
  • Comply with all medical staff bylaws, rules, regulations, policies, procedures, and protocols, as well as Hospital bylaws, rules, regulations, policies, procedures, and protocols.
  • Adhere to all applicable federal, state, and local statutes, regulations, and other legal requirements, as well as any accreditation standards related to the Hospital and its Program.
  • Perform other services as reasonably assigned by the Program's Service leadership.
  • Conduct oneself in a manner consistent with, and that promotes, the Hospital's Mission and Values at all times.
Qualifications
  • Medical degree (MD or DO) with specialization in Nephrology.
  • Board certification in Nephrology.
  • Valid medical license to practice in the relevant state(s).
  • Experience in telemedicine or remote patient care is preferred.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with a multidisciplinary team.
  • Commitment to providing compassionate and patient-centered care.
