Physical Therapist – Outpatient

Location: Boardman, Ohio (Primary location with coverage at other Southwoods PT sites as needed)

Schedule: Full-Time (40 hours per week) |
No Weekends | Flexible Scheduling

Southwoods Health is seeking a dedicated Physical Therapist to provide patient-centered care within our outpatient network. This role focuses on restoring function and relieving pain through personalized, evidence-based treatment plans.

• Clinical Care:
  Perform comprehensive evaluations and administer medically prescribed treatments to restore function and prevent disability.
• Treatment Planning:
  Develop and adjust customized care plans by monitoring patient progress and functional capacity.
• Collaboration:
  
  Partner with physicians and multidisciplinary teams to integrate physical therapy into the patient’s total healthcare plan.
• Documentation:
  Maintain accurate patient records according to State of Ohio and federal regulatory standards.
• Education & Supervision:
  Provide clear post-op and recovery education to patients; supervise PTAs and students as required.

• Education:
  
  Graduate of an accredited Physical Therapy program.
• Licensure:
  Active Ohio State Physical Therapist license.
• Certification:
  Current AHA BLS certification.
• Experience:
  
  Previous experience in an outpatient setting is preferred.
• Skills:
  
  Strong interpersonal communication skills and the ability to thrive in a team-based environment.
