Southwoods Health at Home, a division of Southwoods Health, is seeking a patient-centered Physical Therapist Assistant to join our home health team. In this role, you will provide vital therapy treatments under the direction of a Physical Therapist, helping patients restore function and regain independence in the comfort of their own homes.

• Clinical Care:
  Implement medically prescribed physical therapy treatments according to the individualized plan of care developed by the Physical Therapist.
• Patient Progress:
  Monitor and record patient responses to treatment, including strength, motor development, and functional capacity.
• Collaboration:
  
  Work closely with the Physical Therapist and the multidisciplinary team to suggest treatment revisions and integrate care.
• Education:
  
  Empower patients and their families by providing instruction on home exercise programs and post-operative recovery protocols.
• Documentation:
  Maintain accurate, timely patient records in accordance with Medicare and facility standards.

• Education:
  
  Graduate of an accredited Physical Therapy Assistant program.
• Licensure:
  Active Physical Therapist Assistant (PTA) license in the State of Ohio.
• Certification:
  Current BLS certification through the American Heart Association (AHA).
• Skills:
  
  Strong interpersonal skills and the ability to provide compassionate care independently in a home setting.
