Registered Nurse Case Manager

Mira Sol Health is seeking a Registered Nurse to join us full-time, Monday through Friday, 8:30-5:00. The successful candidate will be responsible for patient care and coordination with the interdisciplinary team. RN duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s physical, emotional, and spiritual needs. Previous hospice case management or home health experience is a plus but not required. Dependable transportation is a must. We look forward to meeting you!

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered
• Handle case assignments
• Facilitate multiple care aspects (case coordination, information sharing, etc.)
• Help patients make informed decisions by acting as their advocate
• Develop effective working relations and cooperate with the interdisciplinary team throughout the case management process
• Required documentation - must be comfortable using an iPad
• Adhere to professional standards as outlined by protocols, rules and regulations

• At least 1 year of experience as a RN
• Excellent organizational and time management skills
• Familiarity with professional and technical emerging knowledge
• Problem solving skills and ability to multi-task
