River Terrace Retirement Community is seeking a passionate and detail-oriented Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator) to join our skilled nursing team in Bluffton, Indiana. This full-time role offers the opportunity to make a meaningful impact by ensuring high-quality resident care through accurate assessments, regulatory compliance, and effective care planning.
Your Role & ResponsibilitiesAs the RNAC / MDS Coordinator, you will:
Conduct routine resident assessments according to schedule and as needed for condition changes, hospital discharges, and other clinical events
Accurately observe, assess, and communicate changes in resident condition to appropriate interdisciplinary team members
Collaborate with Social Services, Activities, Dietary, and Nursing to ensure timely completion of assessments and Care Area Assessments (CAAs) with appropriate documentation
Complete MDS sections, quarterly reviews, CAAs, and care plans in compliance with state and federal regulations
Develop, update, and monitor individualized resident care plans based on assessment findings
Prepare and distribute the MDS schedule to the interdisciplinary team
Transmit MDS data to the state weekly (or as required) and maintain organized, accurate records
Registered Nurse (RN) license preferred; LPNs with strong MDS experience will be considered
Minimum of 5 years of experience as an RNAC / MDS Coordinator
Prior experience working as a Coordinator in a skilled nursing or long-term care setting
Strong knowledge of MDS processes, assessment timelines, CAAs, and care planning requirements
