Physical Therapist

Communi Care Rehabilitation, a member of the Communi Care Family of Companies, has a full‑time opening for a Physical Therapist (PT) at Mercer Healthcare Center. Our therapists are in‑house team members, and this role offers the chance to make a direct impact on residents through therapy.

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Valid or pending state Physical Therapy license

• Evaluate, review, and screen patient/resident medical charts and evaluations under physician order and develop a treatment plan in accordance with patient needs.
• Implement treatment plan through direct treatment, as well as educate the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend or lead interdisciplinary meetings to relay clinical findings and make recommendations to other rehab team members, as well as conduct staff/family training and education when appropriate.
