Under the direction of Clinical Operations, the NP diagnoses and treats acute, episodic, or chronic illnesses as part of a healthcare team within Senior Care Facilities. This position will be responsible for servicing facilities in the Blue Island, Tinley Park, Oak Lawn areas.

• Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
• Diagnose or treat acute health care problems such as illnesses, infections, or injuries.
• Recommend diagnostic or therapeutic interventions.
• Order, perform, or interpret the results of diagnostic tests.
• Maintain complete and detailed records of patients' health care plans and prognoses.
• Prescribe medications.
• Educate patients about self‑management of acute or chronic illnesses.
• Complete full patient consenting process.

• Current and valid NP/PA licensure in IL
• Access to insured, reliable transportation
• 6 months of prior NP/PA work experience

• 2+ years Nursing Home Experience
• 2+ years of Pulmonary experience
• 2+ years of prior NP/PA work experience
