As a Registered Nurse with Geisinger, you can expect this role to involve providing and directing nursing care to patients in their homes, coordinating total plan of care with other health professionals, and maintaining continuity of patient care.
Responsibilities- 1. Clinical Competence
- Initiates, develops, implements, and revises plan of care in collaboration with physician and other health professionals.
- Supervises care provided by home‑health aides and licensed practical vocational nurses, assigns tasks per regulations, and provides required supervisory visits.
- 2. Documentation and Care Delivery
- Provides high‑quality clinical services within scope of practice and infection control standards.
- Accurately and timely completes comprehensive assessments (OASIS) including medication reconciliation.
- Documents patient visits per policy, payer requirements and syncs timely per LHC policy.
- 3. Quality
- Conducts nursing evaluation visits, ensures patients meet eligibility and medical necessity guidelines, and submits accurate documentation.
- Communicates promptly with agency staff, physicians, patients, and families to ensure quality care and service excellence.
- 4. Professionalism (on time, completes training, compliant)
- Arrives daily with a positive attitude, ready to help patients and agency.
- Attends all meetings punctually, participates in case conferences, and maintains video meeting etiquette.
- 5. Teamwork (on call, productivity)
- Works professionally with guidance from Clinical Director and Executive Director, completing tasks timely.
- Assists in orientation of new agency personnel and serves as preceptor to staff and students.
- Participates in on‑call and weekend rotations as needed.
- Adheres to the agency’s utilization management model and engages in continuous improvement initiatives.
- Current RN licensure in state of practice
- Current CPR certification
- Current driver’s license, vehicle insurance, and dependable vehicle or public transportation access
