Care Team Coordinator (Healthcare)
This is a fast‑paced administrative position providing support for assigned providers while managing a high volume of patient calls into our clinical department. The ability to prioritize and multi‑task efficiently is a must. Attention to detail is key.
Full‑time, 8:00 AM–4:30 PM hybrid position. Monday–Thursday in our Bloomington clinic;
Friday remote.
Preferred:
One year experience in a clinical setting.

- Ensure accuracy of patient data
- Strong written and verbal communication

- De‑escalate conflict situations
- Answer phone calls, take and deliver messages to providers and patients via phone, verbally, or electronically.
- Maintain close communication with the provider and Care Team Nurse.
- Prepare necessary paperwork in a timely manner (prior authorizations, FLMA forms, work ability forms, orders/referrals, work notes).
- Continuously improve work processes, environment, and overall experience to serve patients and meet clinic goals.
- Manage cancel/fail encounters, review history accurately, report to provider, and mail letters per provider response.
- Effectively manage and maintain providers’ appointment schedule, ensuring schedules are full and appropriate appointment types are maintained.
- Provide coverage and serve as resource for Care Team Support as needed.
- Review personal desktop, flags, and fax/scanning folder in a timely manner.
- Efficient handling of clinical calls.
- Schedule patient appointments.
- Ensure prepared forms and paperwork are processed timely and accurately.
- Coordinate scheduling ancillary services such as EEG, EMG, MRI, LP, etc.
- Professionally represent the clinic and strengthen communication and relationships with patients.
- Comprehend and follow provider orders.
- Follow rules and guidelines for appropriate chart documentation.
- Document accurate messages according to phone note protocol.
- Synchronize appointments to ensure results are available at time of follow‑up.
