- What You’ll Do
As a Home Health RN, you’ll care for patients where they live — helping them recover, remain independent, and avoid hospitalizations.
You will:
Perform start-of-care visits and routine nursing visits
Complete patient assessments and develop individualized care plans
Provide education to patients and caregivers on diagnoses, medications, and treatments
Monitor progress and adjust care plans as needed
Coordinate care with physicians, therapists, and interdisciplinary team members
Document visits and clinical findings in the electronic medical record
Identify changes in condition and communicate appropriately
Ensure compliance with HIPAA and safety standards
- Support for Nurses New to Home Health
We know home health can feel intimidating if you’ve never done it before — you will not be sent out alone unprepared

Many of our strongest home health nurses came from med-surg, SNF, rehab, clinic, or hospital settings

- What We’re Looking For
Active, unrestricted RN license
At least 1 year of nursing experience in any clinical setting (hospital, SNF, clinic, rehab, etc.)
Willingness to learn and work independently
Reliable transportation, valid driver’s license, and auto insurance
Ability to travel within the branch coverage area
Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
Preferred (not required):
Home Health or Hospice experience
Experience with Home Care Home Base (HCHB)
ASN or BSN
