This is a hands‑on leadership role — not a desk job. You’ll guide and support field staff while overseeing quality patient care.

• Supervise and support RNs, LPNs, aides, and therapists

• Oversee patient plans of care and coordinate services with physicians

• Review documentation and OASIS for accuracy and compliance

• Lead case conferences and provide clinical coaching

• Assist with hiring, onboarding, and staff development

• Step into the field when needed to support care

• Leadership without a full patient caseload

• Active RN license (state specific)

• Associate Degree in Nursing (BSN preferred)

• 2+ years Home Health or Hospice RN experience

• Supervisory/preceptor experience preferred

• Reliable transportation and local travel

• Must be able to lift, pull, and push 50-100 lbs
