- What You'll Do

As a Registered Nurse in Home Health, you will:

* Deliver skilled nursing care to patients in their homes

* Complete comprehensive patient assessments and develop individualized plans of care

* Serve as the primary nurse for your caseload-assessing, planning, coordinating, implementing, and evaluating care

* Perform initial evaluations, routine visits, and re-evaluations based on patient needs

* Monitor and document patient progress, changes in condition, and outcomes

* Communicate significant findings and care updates with physicians, clinical leadership, and the interdisciplinary team

* Revise care plans as patient needs evolve

* Ensure compliance with HIPAA and all regulatory standards

- What We're Looking For

Required:

* Current, unrestricted RN license in the state

* At least 1 year of clinical nursing experience

* Ability and willingness to travel within the Bloomington territory (approximately 50%)

* Valid driver's license, reliable vehicle, and current auto insurance

Preferred:

* Associate's or Bachelor's Degree in Nursing

* Previous Home Health or Hospice experience

* Experience with Home Care Home Base
