Location: University Hospital/Simon Cancer Center
Position: Clinical Nurse - Ambulatory Navigator
Shift: Monday - Friday 8am-4:30p with some flexibility in schedule as the 40 hour work week is maintained;
No weekends, No Holidays
The Clinical Nurse - Ambulatory Navigator coordinates the care needs of assigned patients and develops comprehensive plans to manage care delivery across the patient care continuum from referral to the Bone Marrow/Stem Cell Transplant and Immune Cell Therapy Program to their transplant or CAR‑T therapy. This position partners with patients and their primary hematology/oncology/transplant physicians to develop customized care plans based on individual needs and preferences.
They collaborate with physicians, nurses, health professionals, social workers, and others to ensure appropriate tests and treatments are delivered in a timely fashion in order to get patients to their transplant in a timely manner. Acts as an advocate for the patient, balancing care needs and financial considerations to ensure efficient and effective treatments.
The Clinical Nurse - Ambulatory Navigator plays a vital role in coordinating patient care, developing personalized care plans, and collaborating with interdisciplinary teams to optimize treatment delivery. The role emphasizes patient advocacy, care management, and efficient resource utilization, ensuring a seamless continuum of care tailored to each patient's unique needs.
Key Responsibilities- Coordinate care needs for assigned patients and develop comprehensive care plans across the continuum of care pre‑transplant.
- Partner with patients and primary hematology/oncology/transplant physicians to create customized, needs‑based care strategies.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure timely delivery of tests and treatments.
- Advocate for patients to ensure their care needs are met efficiently and effectively to get patients to the appropriate treatment, transplant, or CAR‑T therapy in a timely manner.
- Balance clinical care with financial considerations to optimize treatment outcomes and resource utilization.
- Associates of Nursing (ASN) required.
- Bachelor of Nursing (BSN) preferred.
Certifications:
- Active Registered Nurse (RN) license in Indiana or Nurse Licensure Compact (NLC) RN license.
- Basic Life Support (BLS) certification through the American Heart Association (AHA).
- Additional advanced life support certifications may be required per department policies.
- Specialty certifications may be required depending on department/provision of care.
- 3-5 years of clinical work experience preferred.
- Knowledge of personal computers, word processing, and database software.
- Understanding of medical terminology, medical record requirements, regulations, and policies.
- Experience in Care management Preferred.
- Experience in bone marrow/stem cell transplant is ideal but not required.
