• The MDS Coordinator ensures the accuracy and compliance of resident assessments.
• Coordinating and completing Minimum Data Set (MDS) assessments for care planning, reimbursement, and regulatory compliance.
• Collaborating closely with interdisciplinary teams to gather comprehensive clinical data and keep resident care plans current.
• Maintaining meticulous documentation and adhering to federal and state guidelines to support quality care delivery and optimize facility operations.
• Contributing to the enhancement of resident outcomes and sustaining the facility’s reputation for excellence.

• Current and valid Registered Nurse (RN) license in Indiana.
• Experience with MDS assessment processes and knowledge of CMS regulations.
• Strong clinical background in long‑term care or skilled nursing facility settings.
• Proficiency in electronic health records and MDS software systems.
• Excellent organizational and communication skills.

Salary: $62,400.00-$99,840.00
