Overview
We are seeking a full-time Licensed Clinical Social Worker (LCSW) for our geriatric mobile practice focused on wellness and disease management in assisted living communities.
Responsibilities- Provide individual psychotherapy using evidence-based methods, conduct assessments, develop treatment plans, and monitor progress.
- Coordinate chronic care management with a multidisciplinary team, facilitate patient education, and advocate for patient needs.
- Collaborate with psychiatric providers, participate in case discussions, and ensure accurate documentation and compliance.
- Travel to multiple assisted living facilities daily, with a requirement for a 2-hour shadowing process for onboarding.
