Full‑time, Monday–Friday, 8:00 am–4:00 pm, with one weekend per month on‑call.

- Active LPN or RN license (RN preferred)
- Experience in geriatrics
- Ideally 3+ years in skilled nursing or long‑term care with strong knowledge of discharge planning and interdisciplinary care
- Excellent communication, organization, and problem‑solving skills
As a Social Services Coordinator, you will coordinate psychiatric and psychosocial services, advocate for residents, and support families through transitions. You will also document social service plans and work with an interdisciplinary team to enhance residents’ day‑to‑day wellness.
