Overview
Our company is an established home healthcare company that has been servicing patients in Chicago Land and Bloomington/Normal areas. We are seeking a high-energy, self-initiating, and caring LPN. Your organizational skills and strong understanding of home health and geriatric care, along with previous experience in home health, will be valued and well compensated.
Main FunctionAdjust and implement the Nursing Care Plan under the supervision of a Registered Nurse.
Responsibilities- Assists the physician and registered nurse by performing specialized procedures.
- Observes and reports to the registered nurse assigned changes in the patient's needs and condition, and documents these changes in the clinical record.
- Observes prescribed procedures for documentation, charting and planning.
- Instructs and counsels patient and family, and interprets services of the Agency to patient and family.
- Gives report regarding problem cases, summary reports, discharges, etc., to the Registered Nurse or Director of Professional Services, weekly or more frequently, as needed.
- Prepares clinical notes accurately and submits documentation in a timely manner, as well as prepares written summaries on a monthly basis and at the time of patient's discharge, when appropriate.
- Schedules workload for maximum efficiency in field and office.
- Maintains the confidentiality of all protected health information whether written or oral which he/she may be exposed to during the course of duties or as a result of accidental disclosures.
- Applies the minimum necessary standards in all matters related to patient's PHI.
- Views, copies, and creates protected health information.
- Discloses protected health information for the purposes of treatment, payment, and health care operation.
- Accesses clinical protected health information limited to patient's charts and referrals.
- Licensed as a Practical Nurse in the State of Illinois.
- One year health-related experience in either a hospital or clinical setting.
- Knowledgeable about current nursing trends and legislative issues.
- Must have graduated from an accredited NLN program.
