Licensed Practical Nurse (LPN) – Full‑time 2nd + 3rd shifts

The 96‑bed skilled nursing center we manage provides in‑patient residential care, short‑term rehabilitation, and specialty services. Each nurse cares for no more than 24 patients, with three aides to support them. We are union‑eligible and offer a competitive wage with an annual increase.
Compensation & BenefitsHourly Wage: $30.91 – $32.91
Shift Differentials:
- 2nd shift (weekdays): +$2.00/hr
- 3rd shift (weekdays): +$2.50/hr
- 1st shift (weekends): +$2.80/hr
- 2nd shift (weekends): +$3.50/hr
- 3rd shift (weekends): +$4.20/hr

Responsibilities (Job Duties)- Assist leadership with 24‑hour accountability for resident care.
- Perform direct patient care to residents according to physicians’ orders and recognized nursing procedures, standards, and administrative policies.
- Administer medications and treatments as prescribed, and assist with specialized therapies such as infusion therapy, PICC lines, respiratory, and wound care.
- Conduct admissions, including assessments and documentation for all residents.
- Participate in the care planning process and maintain the accuracy of care plans between scheduled reviews.
- Observe and encourage infection control measures in accordance with facility policy and regulations governing infection control practices.
- Protect patient confidentiality in all aspects of care and all communication of information.
- Perform and document assessment of residents including behavior monitoring, physical assessment, and response to specific medications.
- Assist physicians in examinations of residents or treatments being performed by physicians.
- Respond to emergency calls across campus if needed.
- Perform other duties as assigned.
- Graduate of a nursing school in good standing, accredited for LPN licensure.
- Current, unencumbered license to practice in the State of Illinois as a Licensed Practical Nurse.
- Current CPR certification.
- Working knowledge of healthcare delivery in the geriatric, rehabilitation, or skilled nursing setting.
