Shift Detail: Full time, part time, per diem available

• Provides personal care and housekeeping assistance to clients in their homes, following the established plan of care under the direction of the client care coordinator.
• Provides personal hygiene and grooming care to clients.
• Provides meals for clients' nutritional needs.
• Reports changes in clients' condition to the client care coordinator or supervisor.
• Performs routine housekeeping and home management duties.
• Records each client's care on the care plan and submits it in a timely manner.
• Responds sensitively to the multiple, complex needs of a terminal client, including end-of-life care.
• Assists with routine therapy exercises.

• Education:
  
  High School diploma or equivalent, preferred.
• Licensure/Certification: CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services required.
• Experience:
  
  Six months of experience as a Home Care Aide/CNA in a home care agency, preferred.
