Warm Hearth Village is seeking a dedicated and detail-oriented Unit Coordinator to support our nursing and administrative teams in providing exceptional care to residents.
This full-time position plays a vital role in the daily operations of our skilled nursing facility, ensuring smooth communication, accurate documentation, and effective coordination across departments. The Unit Coordinator works closely with the Director of Nursing and Administrator to support resident admissions and discharges, maintain accurate medical records, and uphold compliance with all state and federal regulations.
Key responsibilities include:
- Coordinating resident appointments (medical, therapy, diagnostic), transportation, and daily staff scheduling.
- Greeting and assisting residents, families, and visitors, ensuring a warm and professional atmosphere.
- Managing and maintaining accurate resident medical records, charts, and documentation in accordance with facility policies.
- Preparing and distributing daily reports, staffing documentation, and administrative schedules.
- Assisting with admissions and discharges by processing documentation and ensuring records are complete and compliant.
- Performing regular audits of medical records and assisting with quality assurance initiatives.
- Ensuring accurate data entry in systems like Matrix Care and assisting with various reporting tasks for leadership.
- Ordering and maintaining unit medical and office supplies. Upholding all safety, infection control, and emergency preparedness protocols.
- Acting as a key communication liaison between residents, family members, providers, and internal care teams.
- A comprehensive knowledge of Microsoft Office, excellent clerical skills, and familiarity with medical records and terminology.
- Experience working in a hospital, assisted living facility, or long-term care facility.
- Outstanding organizational skills, dependable, detail oriented, and ability to manage multiple concurrent tasks.
- Certification as a nursing assistant by the Virginia Board of Nursing preferred or the willingness to obtain certification.
The starting salary for this full-time position is competitive and based on experience.
