We have an exciting opportunity for a full time permanent Social Worker to become a part of our Neighbourhood Health and Social Care Team. The successful candidates will join our community based multi-disciplinary teams located within health centres in Blackpool. They will work collaboratively to ensure effective health, care and support assessments and services are implemented for people and carers to remain well in the community.
The Social Work role in this area is varied and supports our health colleagues with a wide range of tasks. From completing assessments in line with the Care Act 2014 and multi-disciplinary meetings.
You will need to be passionate about supporting our community in Blackpool, be committed to your role as a Social Worker and keen to grow and develop in line with Social Work England protocols.
